Our primary office is located in Lancaster at the Lancaster General Health Crooked Oak outpatient facility, on the upper level. Some of our providers also see patients in Lebanon and Elizabethtown.

Off Eden Road—Third building on the left
From Route 30, take exit for Oregon Pike north. Drive one-tenth of a mile north on Oregon Pike to intersection at Eden Road. Turn right onto Eden Road (at the Sheetz gas station), and proceed past the Best Western Hotel/Eden Resort. Continue a half-mile to the intersection of Crooked Oak Drive and turn right. Our office complex is the third building on left.
Please use the main entrance on the upper level of the parking lot.
The Center for Spine Care – Lower Level Entrance
The Center for Spine Care is located in NeuroScience & Spine Associates’ main office on the lower level. For directions to the Center for Spine Care, follow the above directions for our main office, but use the lower entrance (entrance with blue canopy). After going through the entrance proceed straight down the hallway through the door to the Center for Spine Care lobby.
NOTE: Only follow these directions if you are scheduled for a procedure at the Center for Spine Care.
Physical Therapy – New Entrance
Our Physical Therapy office is located on the left-hand side of the LG Health Crooked Oak building. For directions to Physical Therapy, follow the above directions for our main office, but park to the left of the building and enter through the doors marked Physical Therapy. There is signage to help guide you to this new entrance.
